Integrated Community Services provide support and training/habilitation in community living service categories to adults age 18 and older who reside in a living unit of a provider-controlled ICS setting (e.g., apartment in a multi-family housing building). ICS can be delivered up to 24 hours per day in the person’s living unit or in the community.
**Must have 245D License and Setting Capacity Report must be approved before enrolling**

Assistance provided to a person or their primary caregiver to help maintain a clean, sanitary and safe environment.
Chore services include, but are not limited to the following activities:
Rent a dumpster or dispose of trash.
Exterminate and provide pest control, limited to a reasonable number of treatments required to alleviate the pest problem.
Perform general indoor and outdoor home maintenance (e.g., lawn care, snow removal).
Deliver groceries when the delivered products meet the majority of the person’s total grocery needs for at least seven days.
Perform heavy household chores (e.g., secure loose rugs, wash floors, windows and walls).
Move or remove large household items and heavy appliances to provide safe access to and exit from the home.
Pack the person’s belongings.
Rearrange or secure household items to prevent injuries or falls.

Covered services include informational, emotional, and physical support for pregnancy, labor and delivery, and postpartum birthing people. This also includes spiritually and culturally responsive care.

Assisted living is a housing option for people for who cannot or who choose not to live by themselves. Assisted living facilities provide a group living style and typically cater to an older adult population.
This type housing setting offers not only a place to live, but also help for those who need it. Help offered can vary widely by facility. Many assisted livings offer meals for purchase for those interested, and most assisted livings offer a variety of social programs as well.

House must be vacant throughout the licensing process
Only residents can live in the home after license approval
License for 5 in a 4 bedroom home ONLY if one bedroom room is at least 150 sq ft of living space & etc
Relocations can only be completed for the following:
Facilities with residents 5 or less
Can only relocate within the geographic boundaries of the municipality in current facility
Allowed once in a 3 year period. Any exceptions must be approved by MDH
Cannot be refunded or transferred if not in compliance with physical environments

Home Care License authorizes providers to offer non-skilled, supportive services like assistance with activities of daily living, medication reminders, and housekeeping.
