All Application Requirements

Incorporate

Incorporate your business through the Minnesota Secretary of States Office. Either an LLC or Corporation would be best.

Federal Tax ID Number

Next you will need to acquire a federal tax ID number. This can be done by going to the website irs.gov

State Tax ID Number

Now you need a state tax ID number. That is done through the MN Department of Revenue.

Insurance

All applications will require you to have a minimum of workers compensation insurance and liability insurance. Some additional insurances maybe required, depending on the application.

Time Frames

Each application process has their own time lines.  Here is what you can expect for each application.

  1. 245D – 30 to 36 months
  2. ICS Setting Capacity – varies by county
  3. Provider Enrollments – 30 to 45 days
  4. Assisted Living – 6 to 8 months
  5. Basic Home Care – 60 days
  6. Comprehensive Home Care – 60 days
  7. Supplemental Nursing Staffing Agency – 60 days

Application Requirements

FAQ’s

We recommend you to register your business documents and tax ids prior to the consultation. However, if you need assistance with this process, we charge an additional fee of $75.

What we’ll need for all owners:

  • Full Name
  • Date of Birth
  • Social Security Number

Our processing takes 3-5 business days after all information is gathered.

You will be notified via email when the application is ready for review or submission. 

We send out confirmations once the application has been submitted or faxed in. If you have not received any emails from our agency, we may be waiting for additional information. Please check your emails.

We do not assist with the following:

Name changes and updating business addresses

You will need to amend and update the following: 

  • Secretary of State
  • Federal Tax ID (Call IRS 1-800-829-4933)
  • State Tax ID

We advise contacting an attorney to assist you with these steps.

  • Change of ownership
  • Transfer of ownership
  • Selling/Buying Businesses
